Therapy cost in Malaysia varies widely. Current published examples range from low-cost government or trainee services to private sessions that cost several hundred Ringgit. Ripple Community charges RM50 for a one-hour online mental health screening with a psychologist. Ongoing therapy is separate.
Price matters, but it is not the only detail worth comparing. Ask who will conduct the session, how long it lasts, whether the fee changes by therapist, and what happens if you need a different type of support. Ripple's adult therapy service page lets you enquire about those details before you decide.
How Much Does Mental Health Therapy Cost in Malaysia?
There is no single national therapy price. These current examples show how much the fee can change by setting and practitioner level.
Government community mental health service
Published price example: MENTARI states that the first consultation is free and subsequent follow-ups are RM5, subject to terms
What to check: Appointment process, eligibility, location, waiting time, and whether the service fits your needs
University-based service
Published price example: Fees can range from free supervised trainee sessions to paid sessions with qualified practitioners
What to check: Practitioner role, supervision, session length, availability, and whether you are comfortable with a training setting
Private clinical psychology
Published price example: The Malaysian Society of Clinical Psychology's 2016 guidance gives a broad RM150 to RM450 per hour range
What to check: Current fee, qualifications, location, duration, and what the appointment covers
Ripple Community online screening
Published price example: RM50 one time for a one-hour online screening with a psychologist
What to check: The screening provides a recommendation and is not therapy, a diagnosis, or a written report
Ripple Community usual adult therapy
Published price example: RM250 for the first therapy session and RM200 for subsequent sessions
What to check: Online or in person; confirm the therapist, duration, and current terms
Ripple Community B40 sliding scale
Published price example: RM200 for the first therapy session and RM150 for subsequent sessions
What to check: Submit an individual or family payslip and confirm eligibility before booking
The Ripple prices were confirmed on 7 August 2026. The other prices and availability were checked on 16 July 2026. Provider fees can change. MENTARI also says an appointment is required, although a referral letter is not required to arrange one. Contact each provider directly for the current terms.
Public, NGO, Trainee and Private Therapy Compared
Lower cost does not automatically mean lower care. The routes are different because they use different funding, staffing, eligibility, and service models.
Government services
Government mental health services can be the most affordable starting point. MENTARI is a Ministry of Health community mental health service. Its official FAQ lists a free first consultation and RM5 follow-ups, with terms and conditions.
The trade-off may be location, appointment availability, or whether the service accepts your case. MENTARI advises people to contact the nearest centre for details.
NGO and subsidised services
Some non-profit organisations and social enterprises provide counselling, therapy, peer support, or crisis support at reduced rates. Ripple Community is a social enterprise and offers sliding-scale fees for eligible B40 clients. Ask about eligibility, the current fee, availability, and practitioner type before booking.
Trainee services
Trainee clinics can make therapy more accessible. Sessions are usually delivered by a practitioner in training with supervision. Ask who supervises the work, how privacy is handled, and whether the trainee placement is long enough for continuity.
Private therapy
Private services often offer more choice of time, format, therapist, or specialism. The price may be higher because you are paying the full cost of the practitioner's time and the service around it.
If you are comparing private options, the guide to counsellors, clinical psychologists and psychiatrists can help you choose the right type of professional before comparing fees.
What Changes the Price of a Therapy Session?
The main price differences usually come from the practitioner, appointment length, type of service, and setting.
Practitioner role and experience
A counsellor, clinical psychologist, psychiatrist, and trainee have different training and scopes of practice. Seniority or specialised training may also affect the fee. A higher fee is not proof that someone is the right fit for you, but the role should match the support you need.
Intake length and follow-up length
The first appointment may be longer because the practitioner needs to understand your history, current concerns, goals, and safety. Some providers therefore publish different durations or fees for intake and follow-up sessions.
Do not compare two prices until you know whether the appointments are the same length.
Psychotherapy or psychological assessment
Psychotherapy and psychological assessment are not the same service. Therapy usually involves ongoing conversations and agreed work over time. An assessment may involve interviews, formal tests, scoring, feedback, and a report. An assessment package can therefore cost more than one therapy session.
Online or in person
Some providers charge the same rate for both formats. Others do not. Online therapy can reduce travel time and may make attendance easier, but you still need a private space and a suitable connection. Read the online therapy Malaysia guide before choosing only on price.
Packages and reduced rates
Some services offer packages, trainee rates, or sliding fees. Ask whether a package is refundable, whether you must stay with the same practitioner, and what happens if the fit is not right. A lower per-session fee is not useful if the terms make it hard to change course.
What Does Ripple's RM50 Online Screening Include?
Ripple's RM50 Online Mental Health Screening is a one-hour online session with a psychologist. You can discuss what has been happening and receive a recommendation on whether further assessment or support may help. The screening does not include therapy, a diagnosis, or a written report.
Ongoing therapy is separate. It costs RM250 for the first therapy session and RM200 for subsequent sessions, online or in person. The B40 rate is RM200 for the first therapy session and RM150 for subsequent sessions. Applicants can submit an individual or family payslip for eligibility review.
The screening is intended to help you:
Explain what has been happening and what you hope will feel different.
Ask about confidentiality and what the screening covers.
Receive a recommendation on whether further assessment or support may help.
The screening lasts one hour and is online only. Ask which psychologist will conduct it, when it is available, and what you should prepare before booking.
How to Choose When Budget Matters
Start with the amount you can realistically sustain, not the highest fee you can pay once.
Write down:
the concern you want help with
whether you prefer online or in-person sessions
how often you could attend without creating financial strain
whether you need a specific professional role or formal assessment
the times you are actually available
Then ask each provider the same questions:
Who will conduct the session, and what are their qualifications?
How long is the first session and each follow-up?
Is the quoted price for an intake, a follow-up, or an assessment?
Are reduced rates, trainee sessions, or packages available?
What happens if the therapist is not the right fit?
This makes the comparison clearer than looking at one price in isolation.
Start With the RM50 Online Screening
Ripple Community offers a one-hour RM50 Online Mental Health Screening with a psychologist. It is online only and gives you a recommendation on whether further assessment or support may help. It does not include therapy, a diagnosis, or a written report. Ongoing adult therapy is separate and may be available online or in person at Plaza Kelana Jaya in Petaling Jaya.
Use the RM50 screening enquiry form to ask which psychologist will conduct it, when it is available, and what to prepare.
If cost is the main barrier, ask Ripple about the B40 sliding scale and prepare an individual or family payslip for eligibility review. You can also compare MENTARI, university trainee clinics, and subsidised services. The right starting point is one that fits both your needs and what you can continue.
